 Indications:

• Nasal obstruction
• Need for improved surgical access prior to ESS

Highlights:

• Steps are similar to those for a standard septoplasty performed with a headlight; however, visualization (particularly of the posterior septum) is improved
• Routine septoplasty instruments can be used with the endoscope
• Initial incision is made through mucosa posterior to mucocutaneous junction (referred to as a “high septal incision”)

Keep in Mind:

• Extreme caudal deflections can be difficult to correct
• Best to learn this technique on posterior deflections
